For enrollment:

• A child must be two years old

• The following forms must be completed

  • WEeCare Enrollment form (includes emergency contact information)​

  • Missouri Department of Health and Senior Services enrollment form

  • Medical examination form (with immunization dates)

  • Food program forms

  • Parent handbook agreement form

  • Deposit equaling two weeks' tuition (this goes directly toward tuition)

If your child does not have all shots you will need proper paperwork from the Health Department.

This applies to all children, no exceptions.
