For enrollment:

• A child must be two years old

• The following forms must be completed

  • WeeCare Enrollment form (includes emergency contact information)​

  • Missouri Department of Health and Senior Services enrollment form

  • Medical examination form (with immunization dates)

  • food program forms

  • Parent handbook agreement form

*If your child does not have all shots you will need proper paperwork from the Health Department.
